Badge Request

1. To receive a St. Monica badge, you must complete and submit the form below to request a badge.

2. When we receive your form, your Safe Environment Status is verified by Marilyn Hicks.
Please note that badges are printed AFTER you have met all requirements of the Safe Environment Program.

3. When your badge has been printed, you will be sent an email informing you that your badge is ready for pickup in the School Office between 7:30 a.m. - 4:00 p.m. on weekdays.

4. You may use your picture from last year. If you are a new employee / volunteer or would like a new picture, a picture will be taken in the School Office between 7:30 a.m. - 4:00 p.m on weekdays.
